Motor Vehicle Accidents/Personal Injury
If you have been in a car accident, you can use your Personal Injury Protection (PIP) from your car insurance for Acupuncture treatment. All I need is your car insurance carrier, claim number and amount left in your PIP to be able to treat you.
For Personal Injury Claims I will need the name of the liability insurance company of business involved and claim number for billing purposes.
Workers Compensation Claims
Acupuncture can be covered by your worker's compensation claim. If you would like to receive treatments from me, then you will need to ask for an authorization and treatment plan from the doctor in charge of your claim.
Care for Veterans
Are you a Veteran? If so, then your Acupuncture treatments are 100% covered by the VA. For you to receive treatment you will need to contact your primary care physician at the VA and request an authorization for Acupuncture. Please let your physician know you would like treatments with Erin Heidrich L.Ac, and they will send the authorization to my front desk and we will call to schedule out your treatments.
The Newly Passed No Surprise Act
Upon agreeing to treatment, it is my duty to provide you a good faith estimate of expected charges for items and services to patient ($400 threshold), and to provide a clear estimate of charges and the likely out of pocket cost to the patient. If for some reason your insurance does not cover the cost of your treatment you the patient are responsible for paying my cash rate for each treatment not covered. My cash prices are clearly marked on my fee schedule page.
If I do not take your insurance, you are more than welcome to request a super bill to submit to your insurance company for reimbursement.
This applies to any insurance based out of the mainland.

All no-shows or appointments that have been canceled or rescheduled with in 24 hours of the appointment will be charged up to $95 fee for service.
